I _______________________________ hereby allow my child to participate in the Delhi Cup practice sessions and football games for the DELHI CUP Season 2015-16 at my own risk. I am aware that there is no insurance coverage provided by Delhi Cup and that my child will be covered only under my own insurance. I will not hold Delhi Cup or any of its organizers or volunteers liable for any injury sustained because of participation in the activities of Delhi Cup.
